Retail and Food Service Establishment Permit Application

If you are opening a new facility or when taking over ownership of an established facility a Plan Review Application needs to be submitted to the Health Department before applying for a Food Permit.

Once the plan review is approved, a Food Permit Application can be submitted. Once the Permit Application is approved the facility may begin food delivery and open for business.
